- Periodontal Disease Care (Not Healthy Gums)

Specific Details on Care Membership Program
- Annual Membership fee is due when applying.
- Have your family join and receive 10% off
- Membership is effective at the beginning of the month in which the payment is made.
- Once services are provided, payments will be due.
- The member is responsible to use their benefits by creating appointments within the annual membership. No refunds will be made to the individual.
- Renewal is due every year in the first week of the month.
